Check out this fantastic resource! The site Disability Hub MN has incredible information for individuals, families, and professionals on how to succeed as adults with disabilities. 

Disability Hub MN is a free resource for those living in Minnesota. The Disability Hub MN team has years of experience and incredible knowledge of community and government programs and how those programs work best together for adults with disabilities. The Disability Hub team says, “We can help you create your best life, your way.” The team wants to help individuals solve problems, determine how to best use community and government systems, and plan for the future.

Click through the colorful site or search keywords in the search tool to find specific content. Pages throughout the site include information on managing health, benefits, work, and housing. On the “Top Topics” drop-down menu, there is information on health including medical assistance, prescriptions, and Medicare; housing including affordable housing and housing stabilization; independence including assistive technology, community first services and supports, PCA services, transportation, and waivers; money including SNAP and social security; and other topics including guardianship and alternatives, and the State Medical Review Team (SMRT). 

Under the “Your Options” drop-down menu, more topics are outlined, and clicking on the “Work” link brings up information on how employment keeps individuals involved in the community. A PDF shares more about the national Employment First movement which promotes a framework that all people with disabilities should have opportunities to work in the community. Also outlined on this page is information on exploring work options, work and benefits, finding a job, supports and accommodations at work, youth and work, how employment has worked for others, and how the hub can help. 

Another great page to explore is Disability Hub MN – Adulthood. This page explores the importance of being well-rounded in adulthood and continuing to grow. The Disability Hub team says, “It’s important to continue to set goals, find supports, build relationships, and plan for the future.” The page covers key supports, programs and services, legal resources, and planning and advocacy. 

Also on the page is a “Focus on adulthood” section outlining that for most adulthood is the longest stage in life. In this section, a “Quick guide: Focus on adulthood (PDF)” is linked. The resource, developed by Charting the LifeCourse Nexus, outlines the elements needed for a well-balanced adult life and how to focus on those elements. The elements of adulthood include daily life and employment, community living, safety and security, social and spirituality, healthy living, advocacy and engagement, services and supports, and family unit. The quick guide asks questions like “What are the things I really like or want to do?” and “What kinds of relationships do I have or want in my life?” to help focus on what is important in an individual’s life.
